At your first appointment you will be asked to complete a medical questionnaire. This form is confidential and is a legal requirement when accepting you as a new patient. During your first appointment, you will be able to discuss in detail any concerns you have regarding your dental health.

We will begin by helping you understand the current condition of your teeth and gums. Once you are happy with the proposed treatment plan, we can work with you on the skills to maintain your own dental health, and help avoid the need for frequent restorative treatment.
